How I Can Help
We provide private care coordination services to individuals assist with navigation of

1. Immediate crises (for example: overseeing hospitalizations, discharges, loss of benefits, sudden changes in health conditions, evictions),

2. Manage gradual health declines (assisting with obtaining medical equipment, home care services, evaluating daily needs, developing and implementing plans, addressing safety concerns etc.), and

3. Plan for the future (facilitating conversations about advance directives and long-term care planning).

Our approach ensures practical and emotional support tailored to each individual's unique situation.

Why I Became A Professional Health Care Advocate
The inspiration for Guide2Care™ arose from my own experiences and the realization that there was a significant gap in the healthcare and social service systems for truly personalized and compassionate care. I saw the struggles individuals and their families faced when trying to access the right resources, and I knew there had to be a better way to support them. Guide2Care™ was born out of a desire to bridge this gap and provide comprehensive, tailored support to those in need.

Additionally, we support both local and long-distance caregivers, experienced or new.

With over 10 years of experience in various settings, including Managed Long Term Care Health Insurance, home care agencies, and a multiple sclerosis center, I bring a wealth of knowledge and expertise to Guide2Care. My background also includes extensive work with Multiple Sclerosis, Alzheimer’s and Parkinson’s diseases, government benefits including Medicare, Medicaid, SSI/SSDI, SNAP ensuring compassionate and specialized care for those in need.
