GuideWell Patient Advocates helps patients and families navigate some of the most stressful parts of the healthcare system, especially during hospital and rehab discharges. With more than 14 years of combined nursing and care management experience, we guide families through decisions that often feel rushed, confusing, or overwhelming.

We specialize in:

Hospital-to-home and hospital-to-rehab transitions

Understanding Medicare benefits, discharge notices, and appeals

Preparing families for safe care at home

Communication with providers and coordinating next steps

Support during complex medical or behavioral health situations

In addition to medical and care navigation expertise, our team includes a nurse with a strong psychiatric background and leadership experience in compliance and patient safety. This allows us to support families facing behaviors, mental health concerns, or complicated family dynamics that may affect care decisions.

Our goal is to bring clarity, safety, and confidence to every step of the process so families never feel like they have to manage the system alone.

Why I Became A Professional Health Care Advocate
I’ve been a Registered Nurse for 14 years, working in emergency medicine, long-term care, home care, and community health. Over the years, I saw how often families felt rushed through hospital discharges or left confused about next steps. Many didn’t know their rights, didn’t understand their options, and didn’t feel heard.

I founded GuideWell Patient Advocates to change that.

My work now focuses on transitions of care, Medicare appeals, discharge planning, and helping older adults and their families make informed decisions. I take complex medical information and make it understandable, and I support families through situations that can quickly feel overwhelming.

What motivates me most is making sure patients are safe, supported, and truly cared for — and giving families a partner who knows how the system works.
