How I Can Help
Nurse Case Management & Patient Advocacy Services:
• Coordinate care across hospitals, inpatient rehabilitation, skilled nursing facilities, home health agencies, and outpatient providers to ensure continuity and prevent gaps in care
• Attend medical appointments (in person or virtually) to take notes, ask questions, clarify provider recommendations, and advocate for patient goals
• Develop and oversee hospital discharge plans, including post-acute placement, home services, equipment needs, and follow-up appointments
• Perform medication reconciliation to identify duplications, interactions, omissions, and adherence concerns; collaborate with providers to resolve discrepancies
• Conduct aging-in-place assessments to evaluate home safety, fall risk, cognitive concerns, caregiver capacity, and support service needs
• Manage insurance denials by reviewing determinations, gathering clinical documentation, drafting appeal letters, and tracking deadlines and outcomes
• Identify and coordinate community and healthcare resources, including home health, therapy services, durable medical equipment (DME), transportation, and specialty providers
• Communicate directly with physicians, nurses, therapists, social workers, and insurance case managers to align care plans and resolve barriers to treatment
• Provide ongoing guidance and emotional support to individuals and families managing complex, chronic, or life-changing medical conditions

Why I Became A Professional Health Care Advocate
My commitment to patient advocacy grew from my own family’s experiences navigating complex medical challenges. When my child was born with a rare birth defect, I learned firsthand how overwhelming it can be to coordinate specialists, therapies, and support services — even as a nurse. Years later, supporting my mother through breast cancer showed me again how difficult it is for families to manage appointments, medications, and insurance while trying to make the best decisions for their loved one.
These experiences shaped my belief that no one should have to face the healthcare system alone. I joined Greater National Advocates to ensure that patients and families have a knowledgeable, compassionate partner by their side, someone who can guide them, protect their interests, and help them feel informed and supported throughout their healthcare journey.
