How I Can Help
We serve patients of any age currently receiving medical care in a facility such as a hospital (inpatient, outpatient, or ER), long-term care (LTC), skilled care (SNF), or Inpatient Rehab. We also serve patients receiving outpatient care from one or more providers such as frequent medical appointments, oncology, home health, etc.
