Will attend medical appointments with patients to provide support or to help attain a better understanding of information provided by their medical provider.
Will assist in making appointments for follow up, with specialists or for diagnostic studies as ordered by medical provider to improve patient compliance and assist in closing any gaps in healthcare.
Will conduct family huddles post appointment as requested and with patient consent to relay any pertinent information to family members unable to participate in appointments.
Will assist patient in setting up and using any devices/technology as needed for telehealth appointments.
