How I Can Help
At Northeast Care Management, we look at everyone holistically to find the right solutions. My goal is to be sure that the highest level of dignity, autonomy, and quality of life is achieved.
I offer a free 30-minute consultation to discuss your situation and key concerns. We will discuss how we develop a customized plan to meet your needs. Care Management is a multi-tiered process, and we will work to provide you with a comprehensive plan and recommendations.
The first step is an Initial Assessment and Evaluation
We will conduct an in-depth comprehensive evaluation of the physical, cognitive, and emotional health of your loved one.
Our holistic approach uncovers medical history, care needs, financial status, family dynamics, personal interests, and short- and long-term goals.
Care Plan and Recommendations
We will develop a written plan of care adapted to the individual's needs and goals. We are creative and solution driven to promote and attain the highest level of independence. Based on our findings we will begin to make recommendations. The recommendations will identify support options for care, selection of providers, appropriate transition placements, and state and federal qualifying programs.
Continuing Care Management
Continuing and Intermittent care management is offered on a short term and long-term basis. We can engage in crisis situations, provide advocacy and management of those situations and provide referrals, arrangements, and recommendations to address medical or behavioral changes to meet the needs of the individual. We can provide on-going monitoring and management to reflect, update and communicate the changes and needs that arise to all individuals and professionals involved.

Why I Became A Professional Health Care Advocate
In college, I studied textiles and went on the path of becoming a buyer, designer, and product developer, working for many major companies, travelling the US and overseas. A friend once said that I didn’t seem happy, and I remember that I told her that what I was doing doesn’t really make a difference in people’s lives. That admission resonated for me for several years until an aunt developed dementia and when a family member and I experienced some medical issues, it was then that decided to change my course of direction.
However, I wanted to be educated on a broader scope of how to serve and care for those experiencing different medical problems along with the aging process. I learned about many holistic modalities that support wellness. While working for a large national healthcare company I also went back to school for a master’s degree in Gerontology. I worked in different capacities and management roles over the years including nursing homes and assisted living communities. Our healthcare system can be a daunting experience for those individuals trying to navigate this, even when you are in healthcare as roles are very siloed. Throughout my own healthcare journey, my education, experience, and the people I serve, I learned about how it weaves together.
In 2021, I opened this practice where I am honored to be able to help adults, seniors and their families navigate this personal journey.
