How I Can Help
I help older adults and their families navigate the challenges of aging with confidence, clarity, and compassionate support. Whether facing a recent hospitalization, a new diagnosis, memory concerns, or the increasing responsibilities of caregiving, I provide personalized care coordination and advocacy every step of the way.
My services include attending medical appointments, coordinating communication between healthcare providers and family members, assisting with hospital-to-home transitions, organizing medical information, identifying community resources, being the eyes & ears or the "Rent-a-Daughter", stepping in for families who need a vacation, and helping families evaluate options for aging safely at home or transitioning to a higher level of care when needed.
For families who live out of town or cannot be present every day, I serve as their trusted local advocate—providing updates, monitoring concerns, and helping ensure their loved one receives the care and support they deserve. My goal is to reduce stress, improve communication, and empower families to make informed decisions while preserving dignity and independence whenever possible.

Why I Became A Professional Health Care Advocate
My passion for patient advocacy is both professional and personal. After spending more than 16 years in behavioral health leadership, care coordination, and case management, I also began experiencing the healthcare system from a family's perspective as my own parents aged. When my father underwent a double lung transplant, I witnessed firsthand how complex and fragmented healthcare can be—even when receiving exceptional medical care. My background in behavioral health also allows me to effectively support older adults experiencing anxiety, depression, cognitive changes, or other emotional challenges that often accompany aging.
I learned that successful outcomes aren't driven solely by excellent physicians. They also depend on having someone who understands the patient's wishes, coordinates communication, asks questions, follows through, and helps keep everyone focused on the individual's goals and quality of life. That experience reinforced what I had seen throughout my career: families often need a knowledgeable advocate to help them navigate difficult decisions and connect the many pieces of care.
I founded Cooper Care & Concierge to provide that guidance. My mission is to help older adults age with dignity, remain as independent as possible, and give families the confidence that someone is walking beside them through every stage of the journey. Whether supporting hospital transitions, coordinating care, attending medical appointments, or helping families understand their options, I strive to bring clarity, compassion, and peace of mind during life's most challenging moments.
