How I Can Help
I will be your dedicated partner - in person or virtually - through every step of the healthcare process. I will apply my 25+ years of medical social work experience to ensure that your wishes are heard, respected and prioritized. Some examples of how we can work together include:
Healthcare Power of Attorney: I can be designated as your HCPOA, assuring that your express medical wishes remain the priority in your treatment, allowing your family to focus on you and one another.
Outpatient medical advocacy: I can accompany you to outpatient appointments to ensure information is conveyed thoroughly and accurately.
Inpatient advocacy: During hospital admissions, I can be at your side to amplify your voice and advocate for your needs and your rights during both treatment planning and discharge planning. I can also provide oversight during transitions between levels of care to maximize understanding for you and your family and to minimize gaps in care.
Family communication: I can serve as a primary point of contact within your family and between family and medical providers, ensuring that everyone stays informed. I can facilitate family meetings (in person and/or virtual) when desired.
In-home care: I can coordinate and oversee your home-based care with the goal of maximizing wellness and autonomy.
Advance directives: I can provide expert guidance in completing your desired advance directive documents (including notarization within New Hampshire.) If desired, I can facilitate a family meeting to discuss the implications of your choices. I offer unique insight and support for patients and families who wish to align their care with Catholic doctrine and medical ethics.

Why I Became A Professional Health Care Advocate
In 2005, my husband relied on my experience as a professional social worker when his mother was diagnosed with incurable liver cancer. When she later lost her decision-making capacity due to vascular dementia, my husband was activated as his mother's HCPOA. Eventually, when her care needs exceeded what we could manage at home, we secured placement for my mother-in-law in a highly rated local nursing facility.
When my mother-in-law transitioned to hospice care, her primary care physician expressed his admiration for the care planning and seamless coordination we had carried out. Unlike many families, ours had the benefit of unrestricted, personalized support from a dedicated professional advocate - me - every step of the way.
My husband then spoke the words that have stayed with me ever since: "Every family needs its own personal social worker!"
I have continued serving as a trusted medical advocate for my family and friends, and I have developed my specific passions for geriatric needs and end-of-life care planning. Recognizing that everyone deserves to navigate healthcare with the guidance of an experienced medical social worker, I answered that call and founded Granite Patient Advocates.
