Lively Health Group is an independent, RN-led healthcare navigation and care coordination service serving individuals and families facing complex medical challenges. Led by a Registered Nurse and Certified Case Manager, we coordinate care, advocate for patients, and connect clients with the resources they need. Our personalized approach ensures nothing falls through the cracks, from diagnosis to discharge and beyond.
Lively Health Group was founded on a deep commitment to serving those who are most vulnerable: older adults, children, new mothers, and underserved communities. Rooted in the belief that every person deserves a knowledgeable advocate in their corner, Lively Health Group exists to bridge gaps, amplify voices, and ensure that no one has to navigate the complexities of the healthcare system alone.
As a registered nurse and certified case manager with over 30 years of clinical experience, I provide compassionate, expert guidance to individuals and families navigating complex healthcare systems. My background spans neonatal intensive care, pediatrics, home health, school nursing, managed care, and care coordination — giving me a broad lens to meet clients wherever they are in their health journey.

Oncology Navigation & Support — A cancer diagnosis brings an overwhelming flood of information, decisions, and logistics. I help patients and families understand their diagnosis and treatment plan in plain language, navigate their health plan coverage for oncology care, and connect to practical support resources, including DME, home health services, wigs and appearance support, financial assistance programs, and community organizations, so they can focus on healing rather than paperwork.
Health Benefits Navigation & Insurance Appeals — Helping clients understand and maximize their health benefits, navigate coverage decisions, and fight back when claims are denied. Services include assistance with prior authorizations, letters of medical necessity, insurance appeals, and assistance finding and enrolling in the right plan — including Medicaid, Medicare, Commercial, and marketplace options.
Discharge Planning & Post-Op Care — Ensuring safe, well-coordinated transitions from hospital to home or the next level of care. I work with clients and families before and after surgery or hospitalization to clarify discharge instructions, arrange follow-up care, coordinate home health or equipment needs, and prevent unnecessary readmissions.
Pediatric & Special Needs Advocacy — Supporting children and families navigating school systems, 504 Plans, and IEPs. I bring firsthand knowledge of the special education process and can help families understand their rights, prepare for ARD/IEP meetings, and advocate for appropriate school-based services and accommodations.
Maternal & Newborn Support — Guiding new mothers through postpartum recovery, newborn care, and access to community resources.
Aging & Life Planning — Helping older adults and their families plan ahead with confidence. Services include care options counseling, coordinating care for aging parents, advance directive guidance, and end-of-life planning, so families can make informed, values-aligned decisions before a crisis occurs.
Chronic Disease & Social Determinants of Health — Helping clients manage diabetes, heart and lung disease, autoimmune conditions, behavioral health, neurodiversity, and substance use recovery, with a strong focus on connecting people to the community resources they need.

Why I Became A Professional Health Care Advocate
My path to advocacy is deeply personal. As a nurse and a mother of five, I have spent my life at the intersection of clinical care and family caregiving, two roles that have shaped everything I do professionally.
In 2022, I suffered the devastating loss of my 22-year-old son to fentanyl poisoning. His death changed me forever and deepened my resolve to build something meaningful. My son had a generous spirit and a dream to make a difference. He once bought a dozen roses just to hand them to strangers in a parking lot, simply to brighten their day. That same heart lives on in this work.
I founded Lively Health Group to be the kind of support system I wish more families had, someone knowledgeable, compassionate, and persistent in their corner. No one should have to navigate a complex health crisis alone, and no family should fall through the cracks for lack of guidance. That belief is at the center of everything I do.
