Care Coordination remains as fragmented and deadly as ever.
Health Care consists of various silos and there seems to be no success in getting them coordinated and talking to each other in order to share life saving and cost effective patient protected health information.
Even if you are the best multitasker and organized in control person, being ill, takes away all of this autonomy.
This is when you need someone to fight and advocate on your behalf.
