I help my clients to connect with trusted service providers in navigating their changing needs.
For example, I help older adults or families connecting trusted home improvement services to create a safe environment to prevent the fall, and help mobility in the house. I follow through from the beginning to the end their services. I act as a liaison between service providers and my clients.
The categories can be extensive; healthcare and insurance, financial and legal services, care coordination, finding suitable assisted living or home care, and much more.
I also work closely with Area Agency on Aging to get as much community benefits that are available.
