How I Can Help
As an aging life care manager, I help older adults and their families navigate the medical, emotional, and practical challenges of aging. I take a holistic, individualized approach, beginning with a comprehensive biopsychosocial and environmental assessment, along with a review of medical records, to understand each client’s needs, strengths, and circumstances. Using this information, I develop a personalized care plan that includes recommendations, resources, and a proposed timeline to address both short- and long-term goals. I work closely with clients, families, caregivers, and providers to coordinate services, advocate within the healthcare system, and ensure care aligns with the individual’s wishes and needs so they can age with the support, safety, and dignity they deserve.

In addition to comprehensive care management, services include:
- Medical appointment accompaniment, advocacy, provider communication, and follow-up coordination
- Hospital, rehabilitation, and care transition planning
- Home Safety and aging-in-place assessments
- Recommendations for the appropriate level of care, with collaboration alongside trusted placement specialists when needed
- Coordination and oversight of in-home care, home health, hospice, and other community services
- Medication management support through coordination of private caregiver, nursing services, adaptive medication systems, and physician-led reviews when indicated
- Family education, care conferences, and caregiver support
- Dementia care planning and behavioral support strategies
- Crisis intervention and emergency care planning
- Navigation of Medicare, long-term care, and community resources
- Ongoing monitoring and regular communication with families
- Identification of gaps in care and coordination of appropriate medical, rehabilitation, mental health, financial, legal and community resources

Why I Became A Professional Health Care Advocate
I began my career working in the healthcare system at the acute inpatient rehabilitation and hospital level, where I quickly saw the many challenges and barriers patients and their families face. It became clear how easily vulnerable adults can get lost in the healthcare system without proper advocacy, education, and follow-up support, often leading to preventable complications and frequent hospital re-admissions. I found hospital case management to be largely reactionary to issues that could often be prevented, which made it difficult to feel that I was truly helping people in the long term. This experience inspired me to transition into geriatric care management so I could take a more proactive, community-based approach -- supporting older adults and their families before crises occur and helping them navigate aging with greater stability, guidance, and care.

Additional Skills
Prior to working as a geriatric care manager in the community, I worked as a case manager in both acute hospital and acute rehabilitation settings, supporting patients with diagnoses including amputations, orthopedic conditions, stroke, and brain injuries. This experience strengthened my knowledge of medical and therapy interventions while also giving me a strong understanding of how to navigate the healthcare system - from inpatient care through post-discharge services. It also allowed me to develop expertise in Medicare benefits and patient rights, effectively communicate with insurance companies, and advocate for patients to ensure they receive the services and coverage they are entitled to.
