Icu helper!

Hello! I'm an advocate in dallas. My primary experience has been in the ICU working at a county hospital for people who could likely never afford an advocate. I still work twice a week in hospital to try to economy buffer my income. Today I worked in the MICU and left my shift exhausted and thinking about the moment before intubation when my patient was wide eyed and terrified, while the doctor spoke to her husband outside and I held her hand. My patient was not at all oriented, so despite her protests, the decision rested on her husband. I asked the doctors to speak with him again. I was so busy managing her pressors and keeping her stable, I was not able to step outside and be part of the conversation, nor could I think of any good reasonable argument for them to not leave the room to have it. We ended up intubating her soon after. I feel in my heart she may die tonight, and her last conscious moments may be of terror. Now I'm on my couch thinking about my day, thinking about her, and wishing everyone had an advocate to explain, to comfort, to paint a compassionate picture of what our options actually are. Of course in that scenario, there were only so many hats I could wear and how far I could push.
I've had thirteen clients, and only one ICU advocacy case, I do hope I can find a way to help a few more.

Leah,
Thank you so much for sharing this incredibly compelling story. I'm so very sorry for that patient; I can imagine that terror she was feeling, and am heartbroken for her.
Thank you so much for the work that you do in the county hospital.

Thinking of you, and that patient, today,
AnnMarie
