Hi Xena! I don't do ongoing management for clients, so it sounds like the diabetes situation may not be a fit unless they are looking for help sourcing services, etc. I would say the same would be true for dementia, but it is hard to say without knowing more details about their current needs and what they have done up until this point. Are they connected with any Chicagoland services related to diabetes and/or dementia yet?
