As an Independent Patient Advocate, I assist adult patients and their families navigate their inpatient hospitalization (acute hospital, inpatient rehabilitation, skilled nursing facility) and plan their discharge - in-person in the NYC area. I visit the bedside, complete daily medical chart reviews, explain medical terminology in plain language, and communicate directly with the medical teams to ensure everyone is on the same page. From navigating the transition to a rehabilitation facility or preparing for home care services, I provide the guidance needed to make informed decisions every step of the way. My goal is to support clients/patients and their families in optimizing their healthcare journey, so they can focus on what matters most - their well-being.
