Peter Abraham

I'm Peter M. Abraham, BSN, RN, EOLD, an independent patient advocate and founder of Compassion Crossing, LLC. I work with patients and families who feel lost in a maze of specialists, paperwork, and rushed appointments, especially those facing a serious illness, hospice or end-of-life care, or caregiving for a loved one with dementia. I bring years of hospice and palliative care nursing experience to help you understand your diagnosis, prepare for doctor visits, organize your medical history, and review your medications for interactions or duplicates. I coordinate between your specialists so information stops falling through the cracks, help you build a values-based advance directive, and keep family members updated so everyone stays on the same page. What sets me apart is that I work for you, not an insurance company, a hospital, or a health system. I don't get paid more if you choose one treatment over another, and I have no financial relationship with any provider you see. My only job is to sit on your side of the table.
